QuickBooks OnlineThe Old Colony Y Transition House/Brockton STARR provides a temporary home and services for youths, age 12 to 18, who are transitioning from one living situation to another. Youth are referred by the Department of Children and Families or the Department of Mental Health, and may stay from one night to several weeks, depending upon their particular situation. The Transition House provides a therapeutic environment where individual needs may be assessed, and appropriate services may be provided; utilizing a broad range of agency and service provider collaborations within the community. The staff are sensitive to the traumas and difficulties experienced by the youth in our care. It is always our goal to connect youth with needed services that will continue with them as they move on to more permanent settings; in order to provide a continuation of care and to keep the youth connected to his/her community. We provide a safe, family supportive environment, where youth and families can develop skills to address current needs, and provide them stability as they strive toward their future goals.
Family involvement is a major component of the program and is encouraged through participation in decisions related to the youth, on-site visits, day and overnight passes, and family therapy; as appropriate and determined by each individual youth’s caseworker. During their stay, various services may be provided; from medical and dental services, to individual and family therapy, anger management, life skills, substance abuse awareness, and other educational experiences. All youth attend school in their home community or receive tutoring provided by service providers. Youth in the program are also encouraged to participate in recreational activities facilitated by program staff, and those available within the Y and the community.
The Transition House is committed to providing a safe and nurturing home-like setting, to prepare youth to successfully transition into their next placement; in a residential program, a foster family, or their biological family, as quickly as appropriately possible.
